Provision of information held by Northumbria Police made under the Freedom of Information Act 2000 (the 'Act')
As you may be aware the purpose of the Act is to allow a general right of access to information held at the time of a request, by a Public Authority (including the Police), subject to certain limitations and exemptions.

I would like to submit a Freedom of Information request to obtain the following information about the witness statement forms (MG11) used by your constabulary:
Please supply (blank) copies of any witness statement forms that you have used since 01 January 2010.
Please include the 'contact details' section for each version of a witness statement.
Please indicate the time period when the witness statement(s) was/were in use.

We have now had the opportunity to fully consider your request and I provide a response for your attention.
As already advised, this will be aggregated with your previous request on the subject.
Following receipt of your request, searches were conducted with the Criminal Justice Department of Northumbria Police. I can confirm that the information you have requested is held in part by Northumbria Police.
We have consulted with the most relevant department/s in relation to this request and they have established nothing prior to 2013 is held by them. If we were to provide a definitive response to this request and search force wide to see what, if anything, is held prior to 2013 anywhere in the force, this request would have to have been refused under Section 12 of the Act. Rather than apply this refusal to your whole request, we are providing you what we have been able to ascertain is held and that is attached.
